Talking Book and Braille Service Application Information

All applications should be sent to the Nebraska Talking Book and Braille Service. You can email a scanned copy of your application to nlc.talkingbook@nebraska.gov, fax your application to 1-402-471-6244, or mail your application to:

If you would prefer to receive a paper copy of any of the applications on this webpage, please contact us and we will send you an application form via the U.S. Postal Service. Please make sure to include your name and a postal address in email or voicemail requests.

Individual Application Forms
(For individuals living in their own homes, with family members, or independently in assisted living centers.)

If the applicant is under 18 years of age, a parent or guardian must complete and sign the Parental Acknowledgment for NLS Services and Devices section of the Individual Application.

 

Facility Application Forms
(Institutions—such as assisted living communities, hospitals, public libraries, schools for students who are blind or have a visual impairment, public and private schools, and college or university disability centers—may borrow equipment and reading materials for eligible readers to use individually or in a group setting. If individual residents prefer to receive service directly (in their own name), they need to submit an Individual Application Form.)

Any institution that serves minors (anyone under 18) must complete the Institutional Acknowledgment for NLS Services and Devices section of the Institutional Application.

 

School Application Forms
(For schools or school systems interested in receiving service for use in the classroom for students who are eligible for talking books. When applying, and at the beginning of each school year, please send in an Annual Student Enrollment Form with the names and eligibility reasons of students receiving service in your classroom.)

All schools must have a legally authorized representative complete and sign the Institutional Acknowledgment for NLS Services and Devices section of the School Application in order to begin service.

 

After you have signed up for Talking Book Service, please inquire about our Braille and Audio Reading Download System (BARD). The BARD website has thousands of audiobooks and audio magazine issues available for download. BARD also has many e-braille files, available for use through an NLS-provided refreshable braille display, or a third-party display. The audiobooks can be played using the NLS Digital Players we loan to patrons, a Windows computer, or the BARD Mobile Application on iOS and Android devices.
